Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals.
• Minimum of one year RN experience; hospice or hospital experience preferred.
• Strong clinical assessment skills; ED or ICU preferred.
• Experience with EMR/EHR systems.
• English communication; Spanish preferred.
• Valid driver’s license and transportation per policy.
• Ability to manage multiple priorities and handle emotional stress.
• Team-oriented with professional presentation.

Responsibilities

• Explain services to potential patients/families and address questions with empathy.
• Identify admission objections and provide responses.
• Educate patient/family on admission requirements and immediate needs.
• Complete informed consents and initial assessment per policy.
• Provide information to physicians and initiate Plan of Care.
• Initiate skilled nursing interventions to prevent complications and improve comfort.
• Obtain physician orders and complete documentation.
• Act as company representative at facilities and coordinate referrals.
• Maintain SBAR communications with Care Navigation.
• Arrange safe discharge and manage referrals to service lines.
