Job summary

Chapters Health System is seeking an Admission RN to assess patients at their residence or in hospitals and initiate care within our service lines. The role includes acting as an ambassador of the organization and coordinating with Care Navigation to ensure a smooth transition.

The position requires an active RN license in the working state, at least 1 year of nursing experience (hospice or hospital preferred), and strong communication in English; bilingual in Spanish is a plus.

Qualifications

• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Current RN license in the state where working.
• Minimum of one year nursing experience; hospice or hospital experience preferred.

Responsibilities

• Explain services to potential patients/families and address questions empathetically.
• Identify admission objections and provide responses.
• Educate patient/family on admission requirements and immediate needs.
• Complete informed consents and initial assessment.
• Provide information to Physicians and initiate Plan of Care.
• Initiate skilled nursing interventions to prevent complications and maximize comfort.
• Obtain Physician orders and complete documentation per policy.
• Represent the Company at assigned facilities and coordinate referrals.
• Communicate with Care Navigation to ensure timely follow-up.
