Job summary

Chapters Health System is hiring an Admission RN to assess patients in their residence or hospital settings and initiate care within our service lines. The role acts as a liaison to Care Navigation, ensuring seamless transitions and continuous care for patients and families.

Candidates should hold a current RN license in the applicable state, have at least 1 year of nursing experience (hospice or hospital preferred), and be proficient with EMR/EHR systems. Bilingual Spanish is a plus.

Qualifications

• Current RN license in the state where the employee will work.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems (Electronic Medical/Health Records).
• Strong clinical assessment skills; background in ED or ICU preferred.

Responsibilities

• Complete clinical admissions for all Chapters Health System services.
• Explain services to patients/families and address questions with empathy.
• Obtain physician orders and initiate Plan of Care.
• Complete documentation per policy and ensure timely communication with Care Navigation.
• Facilitate referrals and coordinate with hospitals, physicians, and facilities.
• Support discharge planning and ensure smooth transitions of care.
