Job summary

The Admission RN assesses patients in their place of residency or in hospitals and initiates care into the services of Chapters Health System, serving as an ambassador of the Organization and ensuring a seamless transition to Care Navigation.

Qualified candidates hold an RN license, have at least one year of nursing experience ( hospice or hospital preferred), experience with EMR/EHR, and strong communication skills; bilingual Spanish preferred. A generous sign-on bonus is available on day one.

Qualifications

• Current RN license in the state where employed.
• Minimum 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills and time management.
• English communication required; bilingual Spanish preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete admissions for all services and explain offerings empathetically.
• Coordinate care with Physicians and Care Navigation; initiate Plan of Care.
• Provide skilled nursing interventions to prevent complications and alleviate symptoms.
• Maintain documentation per policy and SBAR communication.
• Represent the Company professionally and comply with regulations.
