Job summary

Affinity Health Management seeks an experienced Admissions Nurse to join our hospice care team in Cincinnati, OH. You will conduct skilled nursing assessments, collaborate with the interdisciplinary team, and support families at the onset of care to ensure comfort and symptom control.

This on-call role requires Ohio RN licensure, hospice experience, and strong communication. You will coordinate care plans, educate patients and caregivers, and ensure adherence to hospice policies and procedures.

Qualifications

• Graduate of an accredited nursing program.
• Current RN license in Ohio.
• Hospice experience required.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Performs initial comprehensive patient and family assessment to determine hospice needs.
• Delivers professional nursing care per the nursing process.
• Writes and initiates plan of care; revises as needs change.
• Administers medications and treatments per physician's plan of care.
• Counsels patient and family and provides health instructions.
• Acts as liaison among disciplines and coordinates patient care.
