Job summary

Affinity Hospice Management LLC is seeking an Admissions Nurse in Ohio to join our interdisciplinary team. You will perform skilled nursing assessments, plan and implement patient care on an on-call basis to maximize comfort, and coordinate care with physicians, case managers, and families.

You will also serve as a liaison across disciplines during admission, provide education, and ensure safety and quality in care delivery, with a focus on symptom control and patient comfort.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in the state of Ohio in good standing.
• Minimum 1 year professional nursing within last 3 years; or BSN with 2 years’ experience in public health, home care, or hospice preferred.

Responsibilities

• Complete initial, comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan with patient, family, and interdisciplinary team as needed.
• Initiate plan of care and revisions as patient status changes.
• Provide health care instructions and support to patient and family.
