Job summary

Affinity Health Management in Ohio is seeking an Admissions Nurse to join the Hospice team. You will conduct initial assessments, collaborate with the Interdisciplinary Team, and coordinate patient care from onset of care to ensure comfort and symptom control.

The role requires an active RN license in Ohio, hospice experience, and 1–2 years of nursing experience. On-call duties, strong communication, and accurate documentation are essential.

Qualifications

• Must hold valid RN license to practice in Ohio.
• Hospice experience is required.
• 1 year nursing experience in last 3 years; or 2 years with BSN in related areas.

Responsibilities

• Perform initial comprehensive patient and family assessment.
• Provide skilled nursing care per nursing process and physician plan of care.
• Develop and revise care plans with patient/family involvement.
• Coordinate admission process as liaison among disciplines.
• Document patient condition and care provided; communicate changes to the team.
