Job summary

Affinity Hospice Management LLC is seeking an Admissions Nurse to join the interdisciplinary team on an on-call basis. The role includes performing skilled nursing assessments, planning and implementing patient care, and coordinating with physicians, case managers, and families to ensure optimal comfort and symptom control under hospice policies.

The position requires an RN license in Ohio and hospice experience, with responsibilities spanning initial assessments, care planning, medication

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice in Ohio.
• One year of nursing experience within the last three years, or a BSN with two years’ experience (preferred).
• Background checks required after offer of employment.
• Excellent observation, verbal and written communication, problem-solving and basic math skills.

Responsibilities

• Complete an initial comprehensive patient and family assessment.
• Develop and revise a care plan and plan of care with interdisciplinary team.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing needs and self-care techniques.
• Coordinate patient care and act as liaison among disciplines during admission.
• Participate in on-call duties and ensure equipment and services are available.
