Job summary
Affinity Hospice Management LLC in Cincinnati, OH, seeks an Admissions Nurse to perform skilled nursing assessment, planning, and care on on-call basis. You will collaborate with the patient, family, clinical manager, case managers and physicians to complete the initial care plan and ensure comfort and symptom control from day one.
The role requires Ohio RN licensure, hospice experience, and strong communication and care-coordination skills.
Qualifications
- Graduate of an accredited school of nursing.
- Registered Nurse license to practice in Ohio.
- Minimum 1 year of professional nursing in last 3 years (BSN with 2 years in public health/home care/hospice preferred).
- Hospice experience required.
- Must have a valid driver’s license and reliable automobile.
Responsibilities
- Perform initial comprehensive patient and family assessment to determine hospice needs.
- Provide nursing care using the nursing process and implement care plans.
- Initiate and revise care plans as patient status changes to achieve comfort and symptom control.
- Counsel patients and families, provide health instructions, and coordinate with the interdisciplinary team.
- Act as Case Manager for assigned caseload and participate in interdisciplinary group meetings.
- Participate in on-call duties and ensure equipment and services are available.
