Job summary

Affinity Hospice Management LLC is seeking an Admissions Nurse to join the Interdisciplinary Team for on-call skilled nursing assessments and care planning at the onset of hospice care.

The role requires an Ohio RN license and hospice experience. You will collaborate with physicians, case managers, and family caregivers to ensure symptom control and patient comfort while coordinating admission processes.

Qualifications

• Must be a Graduate of an accredited school of nursing.
• RN license in good standing in Ohio.
• Hospice experience required within the last three years preferred.

Responsibilities

• Perform an initial comprehensive assessment of patient and family to determine hospice needs.
• Provide skilled nursing care using the nursing process and plan of care.
• Initiate and revise the plan of care as patient status changes.
• Coordinate patient care with the Interdisciplinary Team and physicians.
• Communicate changes and document findings in clinical notes.
