Job summary

Affinity Health Management in Cincinnati, OH, is seeking an Admissions Nurse to join the Interdisciplinary Team. The role involves on-call skilled nursing assessment, planning, and care to maximize comfort for patients and families in accordance with Hospice policies.

The Admissions Nurse collaborates with patients, caregivers, clinical managers, case managers, and physicians to develop the initial care plan and provide skilled interventions and education to patients and families for optimal

Qualifications

• Hospice experience is required.
• Active Ohio RN license in good standing.
• Minimum 1 year nursing experience in the last 3 years; or 2 years with a bachelor’s in nursing with hospice/public health focus.

Responsibilities

• Conduct initial comprehensive patient and family assessments.
• Coordinate care with interdisciplinary team to develop care plans.
• Provide medications and treatments as prescribed.
• Attend on-call duties and ensure equipment and services are available.
• Document clinical notes and communicate with physicians.
