Job summary

Affinity Hospice Management LLC in Ohio is seeking an Admissions Nurse to join the Interdisciplinary Team on an on-call basis, providing skilled nursing assessment, planning and care to maximize patient comfort in line with hospice policies. You will collaborate with the patient, caregivers, clinical manager, case managers, and physicians to begin care planning at admission.

Qualifications

• Licensed Registered Nurse in good standing (Ohio)
• Hospice experience required
• Ability to work on-call as part of an interdisciplinary team
• Strong assessment, planning and care-provision skills
• Excellent observation, verbal and written communication abilities
• Ability to lift and assist patients as needed

Responsibilities

• Perform initial and ongoing nursing assessments for hospice patients
• Develop, implement and revise care plans with the team and family
• Coordinate with physicians, case managers and other clinicians
• Provide patient and family education and support
• Maintain thorough clinical documentation and notes
• Attend interdisciplinary meetings and participate in on-call duties
