Job summary

Affinity Hospice Management LLC in Dayton, OH seeks an Admissions Nurse to join the Interdisciplinary Team on an on-call basis, providing skilled nursing assessment, planning and care to maximize comfort for patients and families.

You will collaborate with clinicians, communicate care plans, and coordinate admissions per hospice policies, delivering symptom control and compassionate support.

Qualifications

• Must have hospice or end-of-life care experience.
• Valid RN license in Ohio.
• Ability to work on-call and manage patient caseload.
• Excellent communication and documentation skills.
• Must be able to lift and transport equipment.

Responsibilities

• Perform initial comprehensive patient and family assessment.
• Develop and revise care plans with the team.
• Administer medications as prescribed.
• Educate patients and families on care and comfort.
• Coordinate with physicians and other disciplines.
• Provide on-call coverage as required.
