Job summary

Affinity Health Management is seeking an Admissions Nurse to perform on-call skilled nursing assessments and care planning in the hospice setting. You will collaborate with patients, families, clinicians, and physicians to initiate care plans and ensure comfort and symptom control across the care continuum.

The role emphasizes comprehensive assessments, care coordination, and education, with duties spanning documentation, communication with physicians, and ensuring a safe care environment.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in Virginia.
• 1+ year of nursing experience in the last 3 years (BSN with 2+ years preferred in public health/home hospice).
• Background checks and references required.
• Excellent observation and communication skills; basic math and nursing competency.

Responsibilities

• Conducts initial comprehensive patient and family assessment to determine hospice needs.
• Provides skilled nursing care and follows the physician's plan of care.
• Develops and revises the care plan with patient/family involvement.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Provides patient and family education and ensures safe, continuous care; coordinates with the interdisciplinary team.
