CorsoCare is seeking a Hospice Admissions RN to lead the admission process for patients entering hospice care, assessing medical needs and developing care plans to maximize comfort. You will educate families about hospice philosophy and available resources, and serve as a primary contact for concerns.

Qualifications

• Current RN license valid in practice state.
• Minimum 2 years of clinical nursing experience, hospice or palliative.
• Excellent communication and interpersonal skills.
• Compassionate, empathetic, patient-centered approach to care.
• Working knowledge of computer software programs.
• Strong assessment and critical thinking with sound clinical judgement.
• Ability to sit for extended periods.

Responsabilités

• Assess patients referred for hospice to determine needs and eligibility.
• Coordinate admission to hospice and support implementation of services.
• Educate patients and families about hospice services and advance care planning.
• Obtain medical records, vital signs, and functional status to support eligibility.
• Assess and manage symptoms; provide interventions as needed.
• Maintain accurate documentation of assessments, plans, interventions, and communications.
• Collaborate with physicians, nurses, social workers, chaplains and other professionals to coordinate the plan of care.
• Provide emotional support and guidance to patients and families throughout the journey.
• Provide crisis support during times of acute symptom exacerbation and ensure confidentiality.
• Participate in training and continuing education.
