Qualifications

• Current, unrestricted RN license in the state of operation.
• 2+ years of experience in hospice, home health, or related healthcare delivery.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Assess potential patients and families for eligibility.
• Maintain confidentiality and safety of patient records.
• Complete admissions and care orders timely and accurately.
• Communicate with physicians, referral sources, patients and families about admission.
• Collaborate with operations, clinical and sales leadership on referrals.
