Job summary

Affinity Hospice Management LLC is seeking an Admission RN to plan, organize, and direct hospice care with a focus on community health education. The role emphasizes home visits, comprehensive patient assessments, and collaboration with an interdisciplinary team to meet patient and family needs.

The RN will coordinate care as needed, provide counseling and education to patients and families, and participate in ongoing quality improvement to enhance hospice services.

Qualifications

• Graduate of NLN-accredited nursing school.
• Current licensure to practice as a registered nurse in the state and CPR certification.
• RN with either 1 year of professional nursing experience in the last 3 years or a BSN with an RN license; 2 years’ experience in public health, home care, or hospice preferred.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement.
• Administer medications and treatments as prescribed; monitor and document outcomes.
• Coordinate patient care as Case Manager when assigned by supervisor.
• Educate patients and families on self-care and health maintenance.
• Maintain accurate clinical notes and communication with physicians and care team.
