Affinity Hospice Management LLC is seeking an Admission Registered Nurse to plan, organize and direct hospice care for patients in homes and community settings. The role requires nursing experience and teamwork with the Clinical Director and interdisciplinary team.

The position emphasizes comprehensive patient/family assessments, care planning, and coordination with physicians to ensure quality hospice services and patient comfort.

Responsabilidades

• Completes initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient status; initiates and revises plans of care as needed.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Teaches patient and family self-care techniques and health instructions.
• Acts as Case Manager coordinating care for assigned caseload.
• Maintains accurate clinical notes and documents changes in pain/symptom management.
• Coordinates with physician and community health professionals to plan care.
• Ensures a safe environment and continuity of care across settings.
• Assists with activities of daily living and supports patient self-sufficiency.
• Participates in ongoing professional development and quality improvement activities.
