Job summary

Affinity Hospice Management LLC is seeking an Admission RN to plan and direct hospice care within patients' homes and communities. The role emphasizes nursing expertise, community health education, and coordination with the interdisciplinary team to meet patient and family needs.

The Admission RN will perform comprehensive assessments, develop and revise care plans, administer medications, and educate families while supporting self-care and comfort.

Qualifications

• Plans and directs hospice care for patients in home/community settings.
• Provides professional nursing care using the nursing process.
• Acts as Case Manager when assigned by Clinical Supervisor.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Develop and revise the care plan based on nursing diagnosis and patient/family needs.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in meeting nursing and related needs.
• Coordinate patient care with the interdisciplinary team and maintain accurate clinical notes.
• Arrange equipment and services as needed while ensuring a safe care environment.
