Job summary

Affinity Hospice Management LLC is seeking an Admission RN to plan, organize, and direct hospice care. The role emphasizes nursing expertise and community health education to serve patients in homes and communities.

The position focuses on patient assessments, planning and coordinating care, teaching families, and acting as Case Manager when needed, reporting to the Clinical Director. This is a front-line clinical role within the hospice team.

Qualifications

• Initial, comprehensive patient and family assessment to determine hospice needs.
• Professional nursing care using the nursing process.
• Develops and revises an individualized care plan with patient and family.
• Acts as Case Manager when assigned by Clinical Supervisor.
• Coordinates patient care and communicates with the physician and care team.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Writes and initiates plan of care; re-evaluates patient and family needs.
• Initiates revisions to the plan of care as patient status changes.
• Coordinates patient care and serves as Case Manager as assigned.
• Teaches patient and family/caregiver self-care and care instructions.
