Job summary

Compassus is seeking a compassionate Hospice Admission Registered Nurse (RN) to assess eligibility, coordinate admissions, and educate families about hospice services. You will work with the Inter-Disciplinary Team to develop an initial care plan and ensure a smooth transition to home-based care.

The role requires an active RN license, at least two years of RN experience (hospice preferred), a valid driver’s license, CPR and BCLS certification.

Qualifications

• Active RN license in the state of employment.
• Minimum 2 years of experience as a registered nurse; hospice experience preferred.
• Current driver’s license and valid CPR/BCLS certifications.

Responsibilities

• Assess patients for hospice eligibility and explain hospice care services to patients, families, and caregivers.
• Collaborate with the Inter-Disciplinary Team to develop initial plan of care and coordinate admission.
• Ensure all patient initial needs are addressed and facilitate transition to home-based care.
