Qualifications

• 1+ years of skilled nursing experience preferred.
• Active state RN license required.
• Current driver's license and reliable transportation expected.
• CPR certification, TB results, and a current physical are required.

Responsibilities

• Conduct timely and thorough assessments to evaluate patient and family needs for care planning.
• Provide direct patient care, including monitoring and administering medications as prescribed.
• Collaborate with the care team to develop individualized care plans.
• Coordinate communication between patients, families, and healthcare providers.
• Educate patients and families on disease management and self-care techniques.
