Job summary

Affinity Hospice Management LLC is seeking an Admission RN to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes comprehensive assessments, care planning, and family education to support patient comfort and autonomy.

The successful candidate will perform nursing duties, coordinate care as needed, and maintain accurate clinical records while collaborating with an interdisciplinary team to ensure high-quality hospice services.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State and CPR certification.
• Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
• Minimum of two (2) years’ experience, at least one of which is in public health, home care, or hospice nursing is preferred.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing; able to lift and transfer patients and equipment.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Develop and initiate the plan of care with nursing actions; revise as needed.
• Act as Case Manager when assigned and coordinate patient care for the caseload.
• Communicate with physicians regarding needs and changes in condition; obtain orders.
• Teach patient and family self-care techniques and health counseling.
• Maintain safe environment and continuity of care for patient and family.
