Job summary

Shalom Hospice of Washington seeks an experienced Admission RN to plan, organize and direct hospice care within home and community settings. The role relies on strong nursing skills and uses community resources to meet patient and family needs.

The nurse will assess, develop and revise care plans with input from families, educate patients, and collaborate with an interdisciplinary team to deliver compassionate, high‑quality end‑of‑life care.

Qualifications

• Licensed RN with hospice or home health nursing experience.
• Experience in community health education.
• Ability to develop and manage care plans with patient and family.

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writing and initiating plan of care; re-evaluating patient and family needs; revising plan of care as necessary.
• Initiates and revises the plan of care based on patient status and needs.
• Uses health data to determine nursing diagnoses.
• Develops care plans with goals based on nursing diagnosis and palliative actions.
• Administers medications and treatments as prescribed.
• Counsels patient and family in addressing nursing needs.
• Provides health instructions to the patient as appropriate.
• Assists patient with activities of daily living and supports self-sufficiency and comfort care.
• Acts as Case Manager coordinating care for assigned caseload.
