Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Minimum of one year of professional nursing experience within the last three years; hospice/home health experience preferred.
• CPR certification is required.
• Excellent observation, verbal and written communication, and problem-solving skills.

Responsabilités

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Implements nursing care using the nursing process and develops a care plan with the patient and family.
• Initiates plan of care and revises as patient status changes.
• Counsels patient and family; provides health care instructions and supports self-care.
• Acts as Case Manager for assigned caseload and coordinates care with the interdisciplinary team.
• Maintains accurate clinical notes and communicates changes to the physician and team.
