Job summary

Shalom Hospice of Washington seeks a qualified Registered Nurse to plan, organize, and direct hospice care in home and community settings. The RN will use the nursing process to assess patients, develop care plans with families, and coordinate services with the interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice in Washington.
• 1 year of nursing experience in last 3 years; or BSN with 2 years in public health/home health/hospice.

Responsibilities

• Complete and update comprehensive patient/family assessments.
• Provide professional nursing care using the nursing process.
• Develop, initiate and revise care plans with patient/family input.
• Administer medications and treatments as prescribed.
• Coordinate patient care as case manager with the Clinical Supervisor.
• Teach patients and families self-care techniques and health instructions.
• Maintain accurate clinical notes and communicate with physicians and interdisciplinary team.
