Job summary

Shalom Hospice of Washington is seeking a dedicated Registered Nurse to lead and coordinate patient care in home and community settings. The RN will assess needs, develop plans of care, administer treatments and educate patients and families.

The role requires strong communication, clinical judgment and the ability to partner with interdisciplinary teams to ensure compassionate end-of-life care within patients' homes.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• Experience in home health, hospice, or public health preferred.
• Excellent observation and communication skills.
• Ability to lift and assist patients as required.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Develops a care plan with patient and family involvement and revises as needed.
• Initiates and adjusts care plans as patient status changes.
• Maintains accurate clinical notes and communicates with physicians regarding patient needs.
• Acts as Case Manager when assigned and coordinates patient care for a caseload.
• Participates in on-call duties and ensures equipment and services are available.
