Affinity Health Management is seeking an Admission RN to plan, organize, and direct hospice care. The RN will be experienced in community health education and will lead care for patients in home and community settings.

The Admission RN will perform comprehensive patient and family assessments, develop and implement plans of care, administer medications as prescribed, and educate patients and families on self-care and health management within a hospice framework.

Formación

• Licensed registered nurse (RN) with hospice knowledge.
• Experience in patient assessments and care planning.
• Ability to educate patients and families.
• Strong communication and coordination skills.

Responsabilidades

• Completes initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Assesses and evaluates patient status and writes/initiates plan of care.
• Regularly re-evaluates patient and family needs and revises the plan of care as necessary.
• Initiates care plans and revises as patient status changes.
• Develops nursing diagnoses from health assessment data and plans goals.
• Administers medications and treatments as prescribed by the physician.
• Counsels patients and families on nursing needs and self-care.
• Teaches patient and family self-care techniques and health instructions.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Completes and submits clinical notes documenting patient condition and care.
