Job summary

Shalom Hospice of Washington is seeking an Admission RN to plan, organize and direct hospice care in home and community settings. The role requires nursing expertise with emphasis on community health and patient/family education.

Qualifications

• Licensed RN with hospice or home health experience.
• Ability to perform comprehensive patient and family assessments.
• Strong communication and teaching skills for patients and families.

Responsibilities

• Performs initial and ongoing comprehensive patient assessments to determine hospice needs.
• Develops and revises plans of care with patient and family involvement.
• Administers medications and treatments as prescribed and documents care.
• Counsels patient and family on nursing needs and self-care techniques.
• Acts as Case Manager and coordinates care for assigned caseload.
• Maintains accurate clinical notes and communicates with the physician and team.
