Job summary

Chapters Health System is seeking an Admission RN to assess patients in the home or hospital settings and transition care to Care Navigation. You will serve as an organizational ambassador, ensuring seamless service delivery and high-quality patient care.

Requirements include active RN license, 1+ year nursing experience (hospice/hospital preferred), EMR/EHR proficiency, strong assessment skills, and a valid driver’s license with reliable transportation. BLS certification is required.

Qualifications

• Current RN license in the state where working.
• Minimum 1 year nursing experience; hospice/hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• English communication; bilingual in Spanish preferred.
• Mobile driver with valid license and auto insurance.
• Reliable transportation for schedule.
• Team-oriented and able to handle emotional workload.
• Ability to perform essential functions including physical tasks.

Responsibilities

• Completes clinical admissions for all services.
• Explains services to patients/families and addresses questions empathetically.
• Identifies admission objections and responds.
• Educates patient/family on admission requirements.
• Initiates nursing interventions to prevent complications and comfort patients.
• Obtains physician orders and completes documentation.
• Represents company at facilities and facilitates referrals.
• Coordinates with Care Navigation for timely follow-up.
• Communicates with SBAR and with after-hours staff.
• Arrange safe discharge and obtain medical records.
• Develops relationships with case managers and physicians.
• Educates on available services with PRRs.
• Provides care for GIP patients in hospital.
• May be required to report to different locations during emergencies.
• Performs other duties as assigned.
