Job summary

Chapters Health System is seeking an Admission RN to assess patients in their home or hospital settings and initiate care within our services. The role acts as an ambassador for the organization and ensures a smooth transition to Care Navigation.

Requirements include an active RN license in the working state, at least 1 year of nursing experience (hospice or hospital preferred), and experience with EMR/EHR systems. A valid driver's license and reliable transportation are required.

Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience working with an EMR/EHR system.
• Strong clinical assessment skills; ED/ICU background preferred.
• English communication (verbal and written); Spanish preferred.
• Valid driver’s license and automobile insurance per company policy.
• Reliable transportation to meet schedule.
• Ability to work in a team environment and manage multiple priorities.

Responsibilities

• Complete clinical admissions for all company services.
• Explain services to patients/families and address questions empathetically.
• Identify admission objections and provide responses.
• Educate patient/family on admission requirements and needs.
• Obtain physician orders and initiate Plan of Care.
• Coordinate with Care Navigation and referral facilities.
• Maintain SBAR communications and timely documentation.
• Support discharge planning and post-discharge coordination.
