Job summary

Agape Care Group is seeking a Hospice Account Executive to serve as the primary liaison between our hospice agency and local healthcare providers, ensuring patients access high-quality hospice services.

The role focuses on building trusted relationships with hospitals, physicians, and referral partners, educating providers about eligibility, and coordinating with clinical teams to ensure smooth patient transitions within a field-based setting in the Texas region.

Qualifications

• Formal education or proven work in health-related field required.

Responsibilities

• Build and maintain relationships with hospitals, physicians, case managers, and referral partners.
• Generate new hospice referrals within assigned territories and existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams for seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
• Maintain accurate referral documentation and communications with care teams.
