Job summary

Agape Care Group seeks a Hospice Account Executive to cultivate relationships with hospitals, physicians, and referral partners, driving referrals and patient access to hospice services. You will educate providers on eligibility and coordinate with clinical teams for smooth patient transitions.

Ideal candidates have healthcare sales experience, strong relationship skills, and a heart to serve patients and families, with field-based duties across territories.

Qualifications

• Heart to serve patients and families; provide compassionate care.
• Associate Degree required or proven work experience in a health-related field.
• 2 years of sales experience in a clinical care setting; hospice preferred.

Responsibilities

• Build and maintain relationships with hospitals, physicians, case managers, and referral partners.
• Generate new hospice referrals within assigned territories and with existing accounts.
• Educate healthcare professionals about hospice services and patient eligibility.
• Coordinate with internal clinical teams for seamless patient transitions into hospice care.
• Identify new market opportunities and strengthen community partnerships.
• Maintain accurate referral documentation and communication with care teams.
