Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

The position offers flexible scheduling within 8am–8pm hours across a 7-day week, with a $10,000 sign-on bonus for external candidates and no on-call requirements. Local travel and strong EMR use are integral to daily duties.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Ger iatric, or Adult-Gerontology PCP
• Active and unrestricted license in the practicing state
• Access to reliable transportation to complete home visits
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct annual in-home health assessments for Medicare Advantage and other members
• Perform history review, med reconciliation, vitals, and physical exam
• Conduct evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members’ PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on diseases, medications, and adherence; address social determinants of health and refer as needed
