Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

The position offers flexible scheduling, no on-call, no weekends, and no holidays. Candidates must hold an active NP/PA certification and state licensure, with the ability to travel to patient homes across assigned regions.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontology Primary Care.
• Current state licensure or ability to obtain by start date.
• Active and unrestricted driver's license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
