Job summary

Optum Home & Community Care, part of the Optum family, is seeking a HouseCalls Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other plan members. This is a field-based role focused on education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible scheduling with no on-call, weekends, or holidays; 8am-8pm coverage across a 7-day week. Eligible candidates must be ANCC/AANP certified and hold an unrestricted CO license, with reliable

Qualifications

• ANCC or AANP Family/Adult/Geriatric/Adult-Gerontology Primary Care certification.
• Unrestricted license in CO or ability to obtain by start date.
• Driver's license and reliable transportation for home visits.
• Ability to perform in-home assessments in varied environments.

Responsibilities

• Perform in-home health assessments including PMH, vitals, and physical exams.
• Conduct screenings and point-of-care testing as appropriate.
• Identify/document diagnoses for care management and planning.
• Communicate findings to the patient's PCP to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address Social Determinants of Health and provide referrals as needed.
