Job summary

Optum Home & Community Care seeks a Nurse Practitioner or Physician Assistant to join its HouseCalls program, delivering annual in-home health assessments to Medicare Advantage members. This field-based role focuses on preventive care, identifying health risks and closing gaps in care in a home setting.

Flexible scheduling is offered with 20-40 hours per week, no on-call, weekends or holidays, and opportunities for growth within an expansive benefits program.

Qualifications

• Active, unrestricted Nurse Practitioner or Physician Assistant certification.
• Active, unrestricted state licensure (or ability to obtain before start).
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to work in varied home environments and perform physical assessments.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and health plan members.
• Perform physical exams, medication reconciliation, screenings, and preventive evaluations.
• Identify and document diagnoses to support care management and treatment planning.
• Collaborate with primary care providers to address care gaps and improve outcomes.
• Educate members on health conditions, medications and community resources.
