Job summary

Optum is seeking a HouseCalls Nurse Practitioner to perform in-home health assessments across Medicare Advantage and related plans. The role emphasizes non-prescribing, education, gap closure, and collaboration with primary care providers, with a strong focus on delivering care directly to patients in their homes.

The position offers flexible scheduling (8am–8pm, 7-day week) with no on-call, weekends, or holidays. Sign-on bonus of $10,000 for external candidates is included as part of the role.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted Colorado license (or ability to obtain by start date).
• Active and unrestricted driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments as needed.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Review past medical history, med reconciliation, vital signs, and physical exams.
• Perform evidence-based screenings and point-of-care testing (as appropriate).
• Identify diagnoses for care management and treatment planning.
• Communicate findings to patients' PCPs to close care gaps.
• Recognize urgent or emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
