Job summary

UnitedHealth Group's Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other plans. This non-prescribing, field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

Flexible scheduling is offered where providers can flex their time between 8am-8pm over the 7-day work week. No on-call, no weekends and no holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, or Gerontic Primary Care.
• Unrestricted licensure in the practicing state with ability to obtain by start date
• Active driver's license and reliable transportation for home visits
• Ability to navigate varied home environments; sit/stand/kneel as needed to perform assessments
• If required, eligibility for independent practice must be pursued within specified timelines

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medications, vital signs, and physical examination.
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to primary care providers to address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
