Job summary

Optum Home & Community Care (UnitedHealth Group) is hiring a Nurse Practitioner to perform in-home health assessments for Medicare Advantage and other health plan members. The role emphasizes preventive care, education, and closing care gaps while enabling autonomy and meaningful patient impact.

Flexible scheduling between 8 AM and 8 PM with 20-40 hours per week, no on-call, weekends, or holidays, supporting a strong work-life balance.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license or ability to obtain before start date.

Responsibilities

• Conduct comprehensive in-home health assessments in a non-prescribing, preventive care model.
• Collaborate with primary care providers to close care gaps and address social determinants of health.
• Provide member education and connect patients to needed resources to stay healthy at home.
