Job summary

Gentiva Hospice is seeking an RN Case Manager to lead nursing care for patients in home-based hospice settings. You will collaborate with physicians, LPNs, CNAs, social workers, and other team members to tailor care plans and provide compassionate support to patients and families.

Responsibilities include comprehensive assessments, ongoing plan updates, education for caregivers, and timely documentation. A safety-sensitive role with emphasis on regulatory compliance and quality improvement.

Qualifications

• RN with a valid license in the state of employment.
• Minimum 1 year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care in home settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Teach caregivers practical training and reassurance for home care.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate care.
