Job summary

Gentiva is seeking an experienced Hospice RN / RN Case Manager to lead in-home nursing care for patients with life-limiting illnesses. You will assess, implement, and adjust plans of care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive service.

Emphasis on education, bereavement support, and patient-centered care. You will document care plans and participate in on-call activities as needed, maintaining compliance and quality

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver nursing interventions and symptom management.
• Coordinate care with interdisciplinary team to develop and update plans of care.
• Educate patients, families, and caregivers about condition, care plans, and end-of-life topics.
• Document timely and accurate clinical records and ensure infection control standards.
