Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering skilled nursing care in patients' homes or wherever they call home. You will perform comprehensive assessments, develop and update plans of care with an interdisciplinary team, and provide education, comfort, and bereavement support to loved ones.

Qualifications

• Active RN license in Missouri or eligible for licensure
• Experience in hospice, palliative care, or home health preferred
• Ability to assess and coordinate patient care across teams

Responsibilities

• Perform comprehensive patient assessments and ongoing RN visits
• Develop and update individualized plans of care with goals for care
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains
• Provide patient, family education and bereavement support
• Maintain accurate clinical documentation and comply with infection control
• Contribute to quality improvement and scheduling initiatives
