Job summary

Gentiva Hospice seeks a dedicated RN Case Manager to lead patient-centered care in the home setting. You will perform comprehensive assessments, develop plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, compliant hospice services.

You will educate families, document care, and support emotional and bereavement needs while maintaining high-quality clinical records and infection-control standards.

Qualifications

• RN license in the state of employment
• Minimum 1 year RN experience in hospice/home health
• CPR certification
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments in home/ambulatory settings
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care including visit frequency
• Coordinate care across interdisciplinary teams
• Educate patients, families and caregivers about disease, self-care and end‑of‑life care
• Document timely clinical records and maintain regulatory compliance
• Support transitions in levels of care and referrals
• Participate in on-call activities as applicable
