• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments per physician's plan; provide comfort care as needed.
• Coordinate patient care as Case Manager when assigned by Clinical Supervisor.
• Communicate with physicians and interdisciplinary team; provide education to patient and family.
• Assist patient with activities of daily living and support self-sufficiency.
