Job summary

CenterWell Home Health seeks a Speech Language Pathologist to evaluate and treat patients in their homes, helping restore communication, swallowing and quality of life.

You will develop the Plan of Treatment, perform assessments, and coordinate care while educating patients and families and maintaining documentation per policy.

A valid license and CPR, plus ASHA CCC preferred; travel and reliable transportation are required. Benefits include medical, dental, vision, and retirement options.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology preferred by ASHA.
• Minimum six months of experience as a speech-language pathologist.
• Home Health experience a plus.
• Current and unrestricted professional license.
• Current CPR certification.
• Good organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in patient homes or facilities.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and apply therapies.
• Record changes in patient condition and report to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation per policy.
• Participate in care coordination and discharge planning.
