Job summary

CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and treat patients in their homes under supervision. You will develop and periodically review the Plan of Treatment and Plan of Care, focusing on disorders of speech, voice, language, hearing and swallowing to improve function.

You will administer diagnostic tests, observe progress, and report changes to the supervisor, while coordinating with patients, families and healthcare staff to enhance communication safety

Qualifications

• Master's degree in Speech-Language Pathology and licensure.
• Experience in home health or rehab settings preferred.
• Excellent documentation and communication skills.

Responsibilities

• Evaluate and provide speech-language pathology services in the home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Administer diagnostic tests and interpret therapy outcomes.
• Document evaluations, visits and progress per policy.
• Coordinate with care team and educate patients/families.
