Job summary

CenterWell Home Health is hiring a Speech Language Pathologist to deliver in-home or facility-based services, focusing on rehabilitation and functional communication goals. You will assess, diagnose and treat speech, voice, language, hearing and swallowing disorders, and contribute to treatment plans.

We expect ASHA certification, at least six months of experience, a valid license, CPR, and a reliable vehicle. The role includes documentation, family training, and care coordination.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months of experience as a speech-language pathologist.
• Current and unrestricted license.
• CPR certification current.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapies including audiology screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Maintain and submit documentation per company policy; write evaluations and progress reports.
• Participate in coordination activities and discharge planning.
