Job summary

CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities. You will develop and review Plans of Treatment and Care, using professional judgment to manage voice, language, speech, hearing and swallowing disorders.

Responsibilities include diagnostic testing, documenting progress, and coordinating care with supervisors and physicians. A valid license, ASHA CCC and CPR are required, with at least six months of relevant

Qualifications

Must meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology by ASHA.
Minimum six months experience as a speech therapist / speech language pathologist.
Current and unrestricted license.
Current CPR certification.

Responsibilities

Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
Observe, record and report changes in patient's condition and response to treatment.
