Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home evaluations and treatment, participate in Plan of Treatment development, and support patients in regaining communication abilities and independence.

You will assess speech, voice, language, and swallowing, administer tests, and document progress while coordinating with the patient, family, and care team. A valid state license and CPR are required.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license in the practicing state.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing with professional judgment.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe and report changes in patient condition and response to treatment to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain required documentation and care plans.
• Participate in care coordination activities and discharge planning.

Skills

ASHA CCC/Speech-Language Pathology or

6+ months SLP experience

Licensed SLP in state
